Our office requires a photo ID for all patients, and a parent or legal guardian with a photo ID to accompany patients who are minors on the patient’s first visit. All medical information will not be given out to anyone other that the patient, once verified, this includes minors who are considered adults once they are pregnant. No one other than the patient can receive appointment times or change appointments other the patient herself. This is the law (HIPPA), if information needs to be shared with family or spouse they must attend the appointments with the patient.
We require a signed medical release before processing any medical records requests. All requests for medical records will incur a fee. There will be a fee of $1.00 per page for the first 20 pages and .50 cents for each additional page. These will be processed within 15 business days after signed request.
We recognize patients may need to cancel or change an appointment but request they provide at least 24 hour notice so we may offer their appointed time to another patient.
If neglecting to show up to your appointments begins to be a pattern the physician may discharge you from the practice.
Please remember that all staff at Marianna OBGYN strives to provide quality care to all patients and treat each patient with respect and dignity, and in return this office expects the same treatment to all staff.
Dr. Orlando S. Muniz is in the “Florida Birth Related Neurological Association.” Dr. Orlando S. Muniz does not carry Medical Malpractice Insurance.
All co-pays are required at the times of service.
Patients that do not have insurance are considered self pay. Payment in full is expected at the times of service.
Patients are expected to make payment on outstanding balances prior to appointments. Please speak with one of our billing clerks at 850-482-6484 to make arrangements.
